Assignment 2 is a skill-building exercise worth 30% of your final grade. Please complete and submit Assignment 2 after completing Units 1–7. A grade of 50% or greater must be achieved for successful completion of the assignment.

Scenario: You are a health administrator in a pediatric diabetes care centre. The quality-assurance (QA) team has reported that patients and their families are expressing frustration and negative health outcomes when pediatric patients (ages 16–20 years) transition to adult care. The main problems expressed were limited access to health-care providers; poor communication between health-care providers and patients; poor glycemic control; increased number of adverse events (e.g., hospitalizations); and development of concomitant conditions (e.g., depression, eating disorders, high cholesterol). Your goal is to study the transition process and provide recommendations to improve services in this area. First, you ask the QA team to provide you with administrative reports related to transitions from your clinic. Then you go to the research literature to find evidence related to this phenomenon and possible solutions or strategies to improve the transition to adult care. You request 15 minutes of time at the next administration meeting to present your findings from the literature review to your colleagues.

  1. Then search the AU library for 2 more articles that evaluated potential strategies or programs for improving transitions to adult care. Choose evidence-based strategies or programs. Add these to the annotated bibliography for a total of 7 annotations. Save the pdf files of the 2 additional articles you chose for the assignment. You will upload these files with the other documents to the assignment dropbox.

  2. With the information, develop a short presentation for other administrators in your centre, briefly describing the issues related to transitions in diabetes care for young adults and suggesting potential, evidence-based strategies to improve health services. Use the published research literature to support the information. Create slides and a script that you will use to talk about the slides. Use citations in your script to show when you are using information from the literature. Use APA formatting for citations. The presentation should be approximately 10 minutes in length.

  3. Create your presentation using a slide show software such as Microsoft PowerPoint or any open-access software such as Google Slides (use a Google Account).

  4. Ensure that the presentation slides and the script that you will use to describe the slides (i.e., what you plan to say) are both included. Upload the presentation and script, annotated bibliography and the 2 pdf articles to the assignment drop box for Assignment 2 on the course website.
